Fees

Cancellation Policy: If you are unable to keep your appointment, please provide notice within 2 business days, or you will be charged the entire fee of your missed appointment.

All payment is due in full at the time of service.

New patients:

Children/Adolescents $350 - $640 (total)

Time is spent with parents alone and with the child alone.  Often, this will be broken into 2 appointments. 
Adults (18+) $350
1 hour
(A one time consultation is the cost of a new patient evaluation + an additional fee if you need a copy of the evaluation or a summary letter prepared.)
Follow up appointments:
 
All established patients
25-30 mins- $180
50 mins- $300

Out of Network

Horizon Blue Cross Blue Shield, Aetna, and United Healthcare plans are accepted for some services. Otherwise, all payments must be made directly from the patient in the form of cash, check, credit, or electronic transfer.  Insurance companies may reimburse a portion of your payment if you have out of network benefits. Our office can provide a "superbill" which you may use to submit claims to your insurance company. Our office does not submit claims on the patient's behalf. 

 

If you are interested in seeking reimbursement from your insurance, it is helpful to call your insurance company and ask what their current rates of reimbursement are for the following service codes which are commonly used:

Initial Intakes: 90792 or 99204 + 90833

30 minute follow up appointment: 99213/4 + 90833

50 minute follow up appointment: 99213/4 + 90836
